Referral

Referrals can be made for all individuals requiring community based rehabilitation services for neurological impairments. Professional services may be contracted for non-neurological diagnoses upon request.

All referrals will be addressed promptly and efficiently by our team of professionals.

Referrals may be made by: physicians, individuals requiring service, family members, insurance companies, case managers, employers, other rehabilitation agencies, etc.

Although Sunnyside Rehab. therapists have extensive knowledge and experience with physical, cognitive, and perceptual rehabilitation following brain and/or spinal cord injuries, other referrals are accepted. Occupational Therapists are available to complete wheelchair and mobility assessments, In-Home assessments, physical demands analyses, ergonomic assessments, attendant care assessments, splinting, and other rehabilitation services. Physiotherapists are available to provide in-home consultation to individuals with neurological or orthopaedic injuries resulting from automobile accidents.
